Outpatient Mental Health Clinic (OMHC)

At H.O.P.E Behavioral Health, our Outpatient Mental Health Clinic (OMHC) is a welcoming space where individuals of all ages, children, adolescents, adults, and families can receive personalized, compassionate care. Our focus is on empowering each person through respect, dignity, and self-determination.

 

We work alongside you to develop a treatment plan that honors your unique strengths, goals, and preferences. Our team, made up of experienced psychiatrists, therapists, and nurse practitioners—partners with you to support your journey to recovery and wellness.

Services include:

  • Comprehensive assessments to understand your needs and strengths

  • Individual, family, and group therapy grounded in evidence-based practices

  • Medication management and psychiatric evaluations

  • Crisis intervention and stabilization

  • Care coordination with schools, providers, and support systems

Our approach is person-centered, strength-based, and rooted in evidence-based practices to help each individual thrive.

Psychiatric Rehabilitation Program (PRP)

Our Psychiatric Rehabilitation Program (PRP) is here to walk alongside you as you move toward greater independence and a stronger connection with your community. We understand that living with ongoing emotional or behavioral health challenges can be tough—and we’re here to support you in building a life that feels meaningful and authentic to you.


Through one-on-one support and engaging activities, we help you grow your life skills, strengthen your community ties, and take charge of your overall well-being.
PRP services include:

  • Community-based activities that foster connection and help reduce isolation

  • Education and support for managing medications, nutrition, and physical health

  • Mentoring and case management to help you overcome barriers and connect with local resources

Assertive Community Treatment / Mobile Treatment Services (ACT/MTS)

At HOPE Behavioral Health, our ACT/MTS Program is designed to meet you right where you are—physically, emotionally, and in your recovery journey. Whether it’s in your home, your community, or a space where you feel most at ease, our team comes to you with consistent, compassionate care.

If you're navigating complex behavioral health challenges, this program offers a flexible, team-based approach that centers on your unique needs and goals. We’re here to support your journey toward wellness, providing tools and encouragement so you can live life on your terms. With a strong focus on recovery, empowerment, and dignity, we work alongside you to achieve what matters most—whether that’s staying out of the hospital, maintaining stable housing, returning to work or school, or simply feeling more in control of your life.

Our Services Include:

  • Personalized, community-based behavioral health support

  • A multidisciplinary care team: psychiatric providers, therapists, nurses, peer specialists, and more

  • Individualized care planning and case coordination

  • 24/7 crisis prevention and intervention support
